- Both Parents (if 15 or younger)
- Completed Application
*Find out if you’re eligible to renew by mail or online through the U.S. Department of State website at travel.state.gov.
Bring a checkbook or money order for the application fees for the U.S. Department of State. Go to travel.state.gov for a list of current fees.
Get a passport photo at an authorized photo facility. Don’t attempt to take the photo yourself.
Have a completed application.
Review your identification and citizenship documents to make sure they are acceptable. Visit travel.state.gov for a list of acceptable documentation.
Check travel.state.gov for current processing times (and keep in mind that with mail time those estimates may be up to 4 weeks longer).

How to plan your in-person visit
If you got your passport at age 16 or older and the passport has been expired for less than 5 years, you do not need an appointment with us.
We can answer questions about the renewal process, but you will send in the renewal application yourself.

To renew an adult passport that's been expired less than 5 years, use the DS-82 on the US Department of State passport website.
If you require an in-person visit to process your passport application (not for adult renewals expired less than 5 years), you have two options:
- Call the library the day you would like to have your application processed. Our same day, call ahead appointments are available Monday through Friday from 9am to 4pm. The Library staff will let you know if a passport agent is available to fulfill your appointment at the desired time.

I am age 16 or older and this is my first U.S. passport
Obtain the DS-11 passport application in one of the following ways:
- Visit the Lemont Public Library Information Desk to pick up a paper application to complete in black ink.
- Print an application from the U.S. Department of State website or use the PDF form filler on travel.state.gov to complete the DS-11 passport application
- NOTE: Be sure to print the application as two separate pages – not double-sided. Then complete the application using black ink.
Requirements for a first-time passport application for ages 16 or older
- Identification: Applicants must have a valid photo ID; a current driver's license is preferred.
- Proof of U.S. citizenship: To prove citizenship, applicants need to provide their previous passport book or card that will be sent with their application.
- Photo: Get a passport photo at an authorized photo facility. Bring this photo to your appointment. Lemont Public Library does not take passport photos. Do not attempt to take this photo on your own.
- Payment: (Visit travel.state.gov for a current list of passport and expediting fees.)
- Applicants must bring a personal check, cashier's check, or money order made payable to the U.S. Department of State to be sent with each individual application to cover the passport fee.
- A separate mandated processing fee of $35 per application is paid to the Library by credit or debit card, cash, or check.
Tips to completing the DS-11 application
Identification:
- Applicants must have a valid photo ID; a current driver's license is preferred.
- Applicants ages 16 or 17 need to provide their own photo identification and preferably appear at the appointment with one parent to demonstrate parental awareness of the passport application.
Proof of U.S. citizenship: To prove citizenship, applicants need to provide one of the following:
- An original or certified copy of the applicant's birth certificate (if you have a birth abstract, you can go to travel.state.gov to see if it meets the requirements for use)
- A Certificate of Naturalization
- A Consular Report of Birth Abroad or Certification of Birth
- A Certificate of Citizenship
- Note: The Dept of State requires that the original or certified copy of citizenship documents be sent with the passport application. Any original documents will be returned to your mailing address after the passport has been processed.
Passport Photo:
- Get a passport photo at an authorized photo facility. Bring this photo to your appointment.
- Note: Visit travel.state.gov and look at the guidelines for passport photos, then if your photos look off, say something. Photo facilities do make mistakes from time to time.
Payment: (Visit travel.state.gov for a current list of passport and expediting fees)
- Applicants must bring a personal check, cashier's check, or money order made payable to the U.S. Department of State to be sent with each individual application to cover the passport fee.
- A separate mandated processing fee of $35 per application is paid to the Library by credit or debit card, cash, or check.
On page 1 of the application:
- Complete the form using black ink only. Do not attempt to correct any mistakes. If you make a mistake, you must start over with a new form.
- Mark which type of passport document you are applying for at the top of the form. Current passport fees can be found on the travel.state.gov website.
- Use the boxes in Question 5 to list your social security number. Applications will not be processed without this information.
- Do not staple your photo or sign the application. This will be handled by the passport agent at your appointment.
- Make sure to use the newest version of the DS-11 application.
On page 2 of the application:
- Question 10 requires information about the parents of the applicant.
- When listing the mother's last name, make sure to use the last name of the applicant's mother at the mother's birth, also known as the mother's maiden or given last name.

I am under age 16, this is my first passport or my current passport has expired or will be expiring

Identification and Parental Relationship:
- The child (children) and both parents must attend the passport appointment and both parents must present a valid photo ID, preferably a current driver's license.
- A parent or guardian with sole custody can attend the passport appointment with the child applicant and must present a valid photo ID, preferably a current driver’s license, and a court order granting sole legal custody to the applying parent.
When both parents are unable to be present at the passport appointment:
- Form DS-3053, Statement of Consent: Issuance of a U.S. Passport to a Minor Under Age 16, must be completed, notarized and signed by the non-applying parent and submitted with the passport application, along with a front and back photocopy of the non-applying parent’s ID.

My current passport was issued at age 16 or older but has been lost or stolen
You must report to the U.S. Department of State that your passport has been lost or stolen before you can apply for a new passport.
You can use one of the following procedures:
- Obtain the DS-64 form (shown at the right) at the Lemont Public Library. Complete this form in black ink and bring the completed form to your passport appointment.
- Print the DS-64 form from the U.S. Department of State website: travel.state.gov. Then complete the form in black ink and bring the completed form to your passport appointment.
- Use the PDF form filler on travel.state.gov to complete the DS-64 form online for electronic submission to the U.S. Department of State.
After you have reported your passport lost or stolen, you can apply for a new passport.
